Whether you are full-time faculty or are from our affiliate faculty pool, we appreciate the dedication and expertise you bring to the task of supervising MSU Denver’s student teachers. The combination of your on-site visits, conferences, and seminars provides the support and oversight necessary to the success of our candidates.
Please explore the links on the sidebar which will take you to forms needed for assessing the student teacher as well as information on procedures and mileage reimbursement.
The Student Teaching Handbook is updated each summer and provides more explanation regarding responsibilities and expectations.
We encourage you to become acquainted with the content of the student and coop teacher pages so you can help facilitate discussion and answer questions that you may encounter during supervision.

Please submit all Observation, Midterm, and Final Reports through Qualtrics.

The following forms are to be used throughout the student teaching placement. All of the documents are in a PDF format that can either be printed and filled out or typed directly into.
|Seminar Schedule Template||This tool is to help supervisors plan the content of their seminars.|
|Student Teacher Information Sheet||Please have your student teachers fill out this form at the beginning of the semester. It is not necessary to turn this form in to the Student Teaching Office.|
|Daily Class Schedule||Please have your student teachers fill out this form at the beginning of the semester. This form is for your own scheduling purposes.|
|Initial Visit||This form is to be used for reporting events of the initial visit to the student teaching assignment. It is not necessary to turn this form in to the Student Teaching Office.|
|Journal Entry||This form is for the Student Teacher to fill out and hand in before each seminar, or as requested by the University Supervisor. This is an optional assignment.|
|Special Education Checklist||Special Education Student Teachers have additional requirements. Please see this checklist to make sure your student has met his/her requirements.|
|Social Studies Student Teaching Evaluation||This form is now optional. You may use it for your own observation purposes, but our office no longer needs to collect it at the end of the semester.|

|Student Teaching Observation Evaluation – Example||The University Supervisor will fill out one Observation Report for each visit made to the classroom. This is a downloadable example that shows the questions on the official online version of the Evaluation Form.|

Congratulations on another successful semester’s completion!
You will receive the Supervisor End of Semester Letter, detailing the final steps you and your student teachers must take to close out the semester. Please be sure to complete the feedback and evaluation forms via the links shared in the End of Semester Memo.
Letters of Recommendation
It is common for students to ask their University Supervisor for a letter of recommendation after their placement is complete. Please fill out the following FERPA Form if grades, GPA, or other information obtained from Banner will be included in this letter. You may use the School of Education Letterhead for these letters of recommendation.
