Know your benefits

Anthem Health Rewards/CHEIBA Wellness Incentives is a program that rewards participating, benefits-eligible employees for completing various health and wellness programs. Participants can earn $25 to $125, for up to $450 total, toward a digital gift card. 

 

Qualifying programs include:  

ConditionCare 

ConditionCare includes one-on-one support from a health care professional to help manage chronic conditions such as asthma or diabetes. To find out more about the program or to sign up, call 866-962-0953. Complete one of the ConditionCare programs and earn $125. 

Future Moms 

Call 800-828-5891 to get personal support from a registered nurse who can help you make healthy choices, follow your doctor’s plan of care and have a safe delivery. Complete the post-birth assessment by 56 days post-delivery and earn $100. 

Tobacco Cessation or Weight Management 

Work with a health care coach, complete the arranged program and earn $125. To find out more or to sign up, call 833-985-8464. 

Health Assessment 

The Health Assessment gives participants a confidential snapshot of their overall health. To take the assessment, and earn $25 in rewards, log in at anthem.com and look for the Health Assessment in the Health & Wellness section.  

Preventive care 

Preventive exams can help participants stay healthy, catch problems early and even save lives. Earn rewards when you report getting preventive care on the Anthem Health Rewards site: 

• Get your annual adult wellness exam and earn $50. 
• Get an annual dental exam and earn $25. 

After participants complete their first Health Rewards activity, they will have an online balance in their anthem.com account. Balances can be redeemed by selecting a digital gift card on anthem.com or the Sydney mobile app. Note: Reward dollars are taxable. Please consult your tax accountant if you have questions.
