Quick Links to Request Forms
Registered Access Center Students: Requesting previously approved accessible furniture is now done by completing the form located in the Surveys tab under the menu section on the left side of the homepage in the Accommodations Hub.
Non-Access Center Students: Please use this furniture request form to request your accessible furniture needs.
Students who have been approved for lecture note accommodations will need to complete the Lecture Notes Accommodation survey within the Accommodations Hub:
- Log into the Accommodations Hub
- Select the Survey link from the navigation menu on the left
- Select Lecture Notes Accommodations
- Complete and submit the survey.
NOTE: If you do not see the Lecture Notes Accommodation option, you have not been approved for the accommodation.
