The Office of Human Resources at Metropolitan State University of Denver has established a new Salary Supplement Form Submission Inbox to ensure that all Salary Supplement Forms are received and processed in a timely manner. Beginning immediately, departments are asked to submit all Salary Supplement Forms here.  

Forms that were originally sent to the Human Resources inbox will still be processed for August, but moving forward all supplements must be sent to the new inbox.

Please keep in mind the following items for Salary Supplement agreements:

  • The form must be filled out and signed by the department before submitting it to HR. 
  • Incomplete Salary Supplement Forms will be returned to the department. 
  • Salary Supplement Forms that are not submitted to HR prior to that month’s submission deadline may/will be processed on the following month’s payroll. 
  • Use the updated Salary Supplement Form here.

HR will continue to provide departments with monthly email reminders for electronic personnel action forms and Salary Supplement Form deadlines.
