Eligible and participating Metropolitan State University of Denver employees and their spouses who are enrolled in Anthem’s medical plan can receive financial incentives in the form of digital gift cards through the Anthem Smart Rewards program.
Incentives are available for completing the following:
- Online WebMD Health Assessment: $25
- Online Digital Health Rewards information: up to $100
- Well-being Coaching (tobacco cessation or weight management): $100
- ConditionCare (for asthma, diabetes, chronic obstructive pulmonary disease, coronary-artery disease and congestive heart failure): $100
- Claims-based annual wellness exam: $50
- Claims-based annual mammogram: $75
- Claims-based colorectal-cancer screening: $75
- Claims-based prostate-cancer screening: $75
- Meru Program completion: $50
Important information:
- Spouses must register for their own Anthem account to earn rewards.
- Participants can choose from a list of retailers or a prepaid MasterCard.
- Participants can track their progress and reward balances on anthem.com or the Sydney Health app.
How to access Anthem Smart Rewards through the member portal:
- Register at anthem.com.
- Go to the My Health Dashboard, then select My Rewards.
How to access Anthem Health Rewards through the Sydney Health app:
- Launch the Sydney Health App.
- Go to the More menu.
- Select My Health Dashboard.
- Scroll down and select My Rewards.
