If you still need to get a vaccine, you can schedule an on-campus vaccination or find a Covid-19 vaccine near you. Why am I being asked to complete this vaccine verification and exemption form?
MSU Denver is asking all students – including those taking face-to-face, hybrid and online courses – to submit the form to assist with our planning over the next two months.
Is requesting this information a HIPAA violation?
No. This form was created by professionals at the Health Center at Auraria who are well-versed in HIPAA. It was also vetted by MSU Denver’s legal team.
I am only taking online courses. Do I still need to fill out the form?
Yes. MSU Denver is asking all students – including those taking face-to-face, hybrid and online courses – to submit the form to assist with our planning over the next two months.
I intend to get vaccinated but not prior to the July 1 date, which is currently the only option. What do I do?
Select the “July 1” option on the form. We recognize that you will not have received your first dose by then, but we’re more concerned with understanding how many of our students intend to receive their vaccination to assist with our planning.
Is this vaccination requirement for the summer 2021 session as well?
No, this requirement is for the fall 2021 semester. However, all unvaccinated individuals taking face-to-face classes during the summer 2021 session will be required to follow all updated health and safety guidelines, which have been informed by federal, state and local guidance.
Will unvaccinated students be allowed on campus?
Unvaccinated students will be allowed on campus but may be asked to follow additional health and safety protocols in accordance with federal, state and local guidance.
How will MSU Denver know who is vaccinated and who is not?
In addition to this form, information provided by the Colorado Department of Public Health and Environment will allow MSU Denver to assess the vaccination status of our campus community.
I am seeking an exemption for a medical, religious or personal reason. I do not see any of those options on the form.
A general exemption process has been established, and you don’t need to provide a specific reason for exemption.
I am partially or fully vaccinated but received my vaccination in another state. What do I do?
No problem. Enter the date of your first dose. Our vaccine and exemptions team will take care of the rest.
I have submitted the form but mistakenly selected the wrong date for my first dose. What do I do?

Is MSU Denver requiring a specific vaccine (ex. Pfizer, Moderna, Johnson & Johnson)?
The two-dose Pfizer, two-dose Moderna, and one-dose Johnson & Johnson vaccines are all acceptable for meeting our vaccine requirement.
