Whether you are full-time faculty or are from our affiliate faculty pool, we appreciate the dedication and expertise you bring to the task of supervising MSU Denver's student teachers. The combination of your on-site visits, conferences, and seminars provides the support and oversight necessary to the success of our candidates.
Please explore the links on the sidebar which will take you to forms needed for assessing the student teacher as well as information on procedures and mileage reimbursement.
The Student Teaching Handbook is updated each summer and provides more explanation regarding responsibilities and expectations.
We encourage you to become acquainted with the content of the student and coop teacher pages so you can help facilitate discussion and answer questions that you may encounter during supervision.

Please submit all Observation, Midterm, and Final Reports through Qualtrics.

The following forms are to be used throughout the student teaching placement. All of the documents are in a PDF format that can either be printed and filled out or typed directly into.
|Seminar Schedule Template||This tool is to help supervisors plan the content of their seminars.|
|Student Teacher Information Sheet||Please have your student teachers fill out this form at the beginning of the semester. It is not necessary to turn this form in to the Student Teaching Office.|
|Daily Class Schedule||Please have your student teachers fill out this form at the beginning of the semester. This form is for your own scheduling purposes.|
|Initial Visit||This form is to be used for reporting events of the initial visit to the student teaching assignment. It is not necessary to turn this form in to the Student Teaching Office.|
|Journal Entry||This form is for the Student Teacher to fill out and hand in before each seminar, or as requested by the University Supervisor. This is an optional assignment.|
|Special Education Checklist||Special Education Student Teachers have additional requirements. Please see this checklist to make sure your student has met his/her requirements.|
|Social Studies Student Teaching Evaluation||This form is now optional. You may use it for your own observation purposes, but our office no longer needs to collect it at the end of the semester.|

Throughout creation and development, the Teacher Work Sample is evaluated by the University Supervisor assigned to the Student Teacher. Successful completion of this component is required by MSU Denver for the Approved Program Verification Form which is submitted for licensure. The TWS requirements have been created based on the Colorado Teacher Quality Standards.
Teacher Work Sample (TWS)
Please read through the three sections required for your TWS. You must include one bibliography that integrates all resources cited across the three sections in your TWS. Utilize the style guide most appropriate to your professional discipline and as approved by your university supervisor.

The following forms will be used during the completion of the TWS. The University Supervisor will discuss them, along with other requirements, during their seminars.

Teacher Work Sample Rubric for All Areas
The TWS will be graded by the University Supervisor based on the following criteria. The rubric should be filled out and submitted to the Student Teaching Office at the close of the semester.
Students must submit their Final TWS to Blackboard in order to be recommended for licensure. Do not upload until you have received permission from your University Supervisor. Please read the Teacher Work Sample Submission Instructions to ensure you are uploading it correctly. You will not receive an email notification when it has been uploaded; it has been successfully submitted if you are able to see it under your profile in Blackboard.

Congratulations on another successful semester's completion!
You will receive the Supervisor End of Semester Letter, detailing the final steps you and your student teachers must take to close out the semester. We sent your students a letter as well, reminding them to upload their Teacher Work Sample to Blackboard and to complete the evaluation surveys. Please remind your students that they must upload their Teacher Work Sample to Blackboard by the given deadline. This Process For Licensure checklist will be helpful in tying up any loose ends!

See below for this semester's mileage reimbursement forms. That these forms may change from semester to semester, so please make sure you are using the most recent version. Since these are processed through Accounts Payable, you will need to print out and sign each document after you have finished filling it out.

Letters of Recommendation
It is common for students to ask their University Supervisor for a letter of recommendation after their placement is complete. Please fill out the following FERPA Form if grades, GPA, or other information obtained from Banner will be included in this letter. You may use the School of Education Letterhead for these letters of recommendation.
