Reporting an Incident

Please choose the correct reporting or referral form below. However, if you have any questions please don't hesitate to contact us if you are unsure about filing a report, our professional staff in the Dean of Students Office will gladly discuss the situation with you and provide guidance on the available options. 

Incident Report: (general conduct)

This form is to be used by students, staff and faculty to report alleged violations of the Student Code of Conduct.

Academic Misconduct Incident Report: (academic integrity)

This form is to be used by faculty members and others to report academic misconduct violations.

CARE Referral:

This form is to be used by any concerned party to share information about a student of concern. The Student Care Center case managers provide one-on-one support and resources. You can also find more about the CARE Team here.

 

After a report or referral is submitted: 

Once a report has been filed, a staff member in the Dean of Students Office and/or Student Care Center will review the information included. Additional documentation, which can be uploaded to a report, including email exchanges, can be very helpful to provide us context.

Typically the next step in this process include contacting the reporter to confirm that we received the report and to gather more information.

Members of the staff aim to resolve matters at the lowest level possible; therefore, conflict resolution services may be used, as appropriate.

Please note that information shared in the reports become a part of students' educational records and therefore are protected by a federal privacy law called the Family Educational Rights and Privacy Act (FERPA). As such, students may have access to their educational records, which includes access to the name of the person filing the report.

If this is an emergency:

In the event of a life-threatening emergency, please contact the police immediately by dialing 911. Note that the Auraria Police Department can be reached by dialing 303-556-5000 from a cell phone or by dialing 911 from any Auraria Campus phone.  Once the immediate health/safety concern has been addressed, please follow up by filing an incident report using the links above.
